Discharge Medicines Service – Salisbury NHS Foundation Trust Update
You are likely to receive DMS referrals from the following Organisations

| Avon and Wiltshire Mental Health Trust | PharmOutcomes referral – PDF attachment | Working towards all in-patients on discharge |
| Great Western Hospital (Swindon) | PharmOutcomes referral | Currently only patients discharged with Compliance Aid, however discussions ongoing to widen out (eg anticoagulants, patients with recent heart attack) |
| Royal United Hospital (Bath) | PharmOutcomes referral | Aiming to refer all patients where the pharmacy team is involved in the discharge |
| Salisbury Foundation Trust (Salisbury) | PharmOutcomes referral | All patients discharged with a Compliance Aid plus patients on certain wards |
| Wiltshire Health and Care | NHS mail | Patients whose discharge from hospital have involved a “step down” stay in a Community Hospital |
| Other Out of County Providers | May be NHS mail or PharmOutcomes referral |
